小便滴滴答答！他尿失禁嚴重 沒用護墊不能睡
（優活健康網陳思綺／綜合報導）忍不住就尿出來！62歲的陳先生，幾年前曾短暫做過攝護腺肥大治療，因忙碌而中止，近一年來陸續出現排尿困難、急尿，有時來不及到廁所就尿下去。近期情況更加嚴重，排尿時用滴的，每次只能尿出少許，且不時有尿液滲出，甚至需要用護墊檔著才不會弄濕外褲，最後連夜間也會尿床，不得不就醫。
3～4成老年人 有漏尿困擾
收治個案的書田診所泌尿科醫師周固指出，尿失禁就是不自主的漏尿，並非只是女性的問題，男性也會發生。據美國2014統計，65歲以上約30～40％的人有漏尿困擾，男性約占5～15％。
上述個案經初步理學檢查，下腹部微凸，無明顯壓痛；肛門指診發現攝護腺肥大，超音波檢查確認餘尿量高達800cc以上（正常約為400cc）。診斷為攝護腺肥大合併尿滯留及滿溢性尿失禁，經攝護腺雷射切除手術後，排尿情況得到改善，且餘尿量恢復正常，不再漏尿。
尿失禁分4種 年紀大是主因
周固醫師說明，滿溢性尿失禁發生原因，為膀胱無法正常排空，當小便累積過滿，又持續增加時，膀胱壓力上升，使得小便溢出；同時因膀胱壓力過高，使尿液無法自腎臟排出，而積聚在腎盂內，甚至膀胱輸尿管發生逆流，造成腎水腫。此個案由於攝護腺肥大，長時間小便困難，造成膀胱失去代償功能，尿液常不自主的滲出，發生在夜間時就與尿床相似，倘未儘早治療，腎功能也會受到影響。
造成男性尿失禁的主要原因包括：攝護腺肥大、攝護腺癌治療後、神經病變（如糖尿病、脊椎受傷、多發性硬化症、中風等）、服用藥物、年紀大於65歲。周固提到，尿失禁可分為四類：
1）應力性尿失禁／是膀胱出口閉鎖不佳造成，當腹部用力，像是咳嗽、大笑等情況時，腹壓增加，壓迫到膀胱就會漏尿。
2）急尿性尿失禁／膀胱過動所致，可能當感到尿意，來不及到廁所便尿出來。
3）滿溢性尿失禁／因膀胱無力或尿道阻塞導致。
4）功能性尿失禁／由於藥物或其他健康問題造成。
周固醫師提醒，小便頻繁，每次只能尿出少量尿液、排尿時要等很久才能尿出、小便無力、要用盡力氣才能尿出、尿完後仍感覺沒尿乾淨、排尿不舒服、腹部持續有腫脹感，倘有以上情況時，應儘早就醫治療，以免對腎臟造成傷害。
